We Accept Most Insurance Carriers

Please have your card with you at the time of your visit. If we file your claim, you are responsible for the deductible and/ or the co-pay at the time of the visit. If we are not a provider for your insurance, if you are not insured or if we do not file with your insurance company, you may be responsible for the entire charge at the time of service. Be sure to check with your insurance provider to ensure coverage before scheduling an appointment.

You are responsible for any balance your insurance does not cover.

If you ever need help with a balance, call us — we will work with you.

The parent or adult who brings the child is responsible for payment due at the visit (copays, deductibles, and coinsurance).

In cases of divorce or separation, payment is the responsibility of the accompanying parent or guardian. Our office is unable to participate in or resolve financial agreements between parents.

If a new problem comes up during a well-child checkup — or your child needs extra testing, medication changes, or a referral — your insurance may charge a separate copay, deductible, or coinsurance for that part of the visit.

If a balance is unpaid 30 days after your statement, we cannot schedule new routine well visits until it is paid or a payment plan is set up. Sick care is still available — we will set up payment arrangements when you schedule.
